    I agree you should try to find a dog dermatologist. Try calling a 24 hour emergency vet hospital (if there is one, I grew up in Alaska so I know how it is to live on a small island, we didn t even have a vet) and see if they have one or know of one. The big vet hospitals usually know the specialists in town. You can also check for a holistic vet since then your at least dealing with someone who isn t going to try and sell you expensive prescription diet junk/death food and really try to find the issue and resolve it.
